New patients are always welcome at Cedar Brook Dental Group! For your convenience, forms are available on our website that you may print and bring along with you to your first visit if you wish! Otherwise we do ask you to please arrive 15 minutes early at your first visit to fill out the necessary paperwork. Insurance Information

At Cedar Brook Dental Group, we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. We are in-network providers with most dental insurance plans. If you don’t see your provider on the list below, please contact our office and we will help guide you in determining your dental benefits with our office.

If you are covered by a dental insurance plan, as a courtesy we will file your claim for you and request to have the benefits assigned to our office. Insurance is an agreement between you, your employer and your insurance provider, therefore most insurance related questions should be directed to them. We know insurance can be confusing, but we will help answer any of your coverage questions and help you file a claim so we can keep a healthy and happy smile on your face!

Financing Information

You can pay for your office visits and any treatments by cash, check, credit card, or debit card at the time of treatment. We accept MasterCard, Visa and Discover in addition to most major dental insurances. For patients without insurance, a 5% discount will be applied to balances paid in full the day of treatment.
